The Town Clerk's Office is responsible for original records of all deaths that have occurred in Concord, including Emerson Hospital. All death certificates are public records. A certified copy of a death certificate may be obtained in person or by mail. The fee is $10 for each certified copy (note that this is a change in rate as of January 1, 2013). There is no fee for burial permits.
Online Requests & Payment
Online death Certificate payments can now be made through your checking account or with a debit or credit card (Visa, Master Card or Discover Card). You will receive your documents via mail (rush delivery available for an extra fee). This secure service is made possible through an agreement with Unibank. A nominal fee will apply to credit card transactions, while there is no charge to pay directly from your checking account.
Office Pick Up

Request By Mail
Send a request stating the name and date of death on the certificate you are seeking. Include a check or money order, payable to "Town of Concord," and a self-addressed, stamped envelope. Mail your request to:
